I tested it with a Mega-SCSI on a fat16 partition. Just grab it with NearDark's ReadIt and run the game with RunIt. There is no need to fix the diskimage then. I did the same with Kyokugen and that works the same. Kyokugen didn't run with getdisk->adapt->start.
I'm searching for this game at the moment, but found out it isn't for sale anymore After sending an e-mail to M-kai he told me that he there is quite a demand for this game from foreign countries. And an investigation of the possibilities to re-distribute the game is an option. He also told me that this is not in his hands We'll just have to wait I guess....Although Kyokugen and Pleasure Hearts are now freely available I would buy an original copy of them if they are redistributed. By the way, on YouTube there are some videos of the games: Kyokugen (1 and 2), Pleasure Hearts (1, 2, 3 and 4).
You can order Kyokugen from MSX World Wide. Just take a look at their webshop. Pleasure Hearts isn't for sale anymore so you can only buy that second-hand. If you can find anybody who is willing to sell...

Just one. He got the original game when it was for sale in Japan years ago.
I thought in Holland Pleasure Hearts was sold by Rinus Stoker from MSX INFO for a long period of time? Not sure......
Rinus only sold (and maybe still sells) Kyokugen. He never sold Pleasure Hearts iirc. Club Hnostar did sell Pleasure Hearts in the past, but not anymore.
