You can add it yourself with basic soldering and electronics skills.
The software does not need to be updated, since you can use the /CS or DI and DO SD card signals to check the activity.
I have not done it myself, so I can't post a tutorial.

I've opened up the cartridge to see if I can add a read/write status led.
I see that the SD-card slots are connected to testpoints P1 till P8

@Guillian can you please give me the signals on P1 till P8 ?

If i have to guess, it is:

1 - SD1 / CS
2 - SD1 / DI
3 - SD1 / CLK
4 - SD1 / DO
5 - SD2 / CS
6 - SD2 / DI
7 - SD2 / CLK
8 - SD2 / DO

can you open the cartridge without damaging ?

No, there are 2 plastic pins that will break, but the label is still undamaged. I have noticed this with all cartridges from the Cartridgeshop.
But superglue can fix this ;-) There is no external damage.

Thanks Guillian, but does the SD2 slot does'nt have a DI signal ? Or is that one not present on the PCB pads ?

DI is common for both SD cards.

I've tested a status-LED on SD1-D0, with the following results:

Test on my Zemmix NEO:
At initializing the SD card, the LED flashes on activity and switches OFF.
When I'm in Dos and issue a DIR command, the LED flashes on activity, but stays ON like DO is still high after finishing reading.

Test on my Turbo-R ST
LED goes on at Boot and doesn't blink at any activity.

Tomorrow I'll hook up my logic analyzer to see what's going on.

It is not used. IIRC, the idea was to add an small description to each ROM. But it was not implemented.

I'm pretty sure it doesn't require any hw support from MFR to do this? Possible implementation is in opfxsd with multirom.rom software parts, right? (Just an idea)

I have made a quick test and seems to work.
Try to connect the cathode to DO and the anode+resistor to 3.3V
