omg!!! I want one of these RIGHT NOW!!!

i am having trouble with the form though. Some fields do not make sense when I run them through Google translate. Any Japanese speaker could lend a hand? I can see the first line is family name and given name, but I don't know what is the second line (Google translate says "phonetic"). Or perhaps there is some email address at which we could reach BEEP Akihabara?

The input box of the second row is how to read kanji letters of the name. Pronunciation of Japanese name is underspecified, so these questionary field is required. I'm sure you just have to input the same letters as name field there.
