It was included in this magazine's tape Info here
The levels are new. It is different to the original game.
I'm not sure if the code listing was also in the magazine or not (then someone can type it in if the tape can't be found).

Apparently there was also a different version of that gamebook (slightly different cover) with the listings in it for those games.
